TV CHOSUN’s Doctor Shin: A Gripping Review of the Brain Swap Saga and Its Dark Ending

[Sports Today reporter Im Si-ryeong] TV CHOSUN’s weekend drama Doctor Shin delivered a jaw-dropping twist in Episode 3, staging a brutal, fate-driven death finale after a botched brain-swap that left Baek Seo-ra’s body inhabited by Song Ji-in’s character.
The March 21 episode opened with the fallout from the experimental procedure: Hyun Ran-hee (Song Ji-in), whose brain now occupies Momo (Baek Seo-ra)’s body, is consumed by a rising sense of dread.
Awake in the recovery room, Momo — in appearance — is asked for her name by Shin Joo-shin (Jung Eui-chan). She requests a mirror, and when Hyun Ran-hee sees her reflection in Momo’s body she breathes a relieved, “If it worked, that’s enough. You did well,” and reaches out. Yet Shin Joo-shin, seeing Momo’s face paired with Hyun’s voice and mannerisms, is visibly shaken to tears. Meanwhile, Hyun — now carrying Momo’s brain — regains consciousness but speaks haltingly and can barely stand. Momo (in Hyun’s body) is unsettled by the sight of herself. Before Shin returns from an overseas seminar, Momo (with Hyun’s brain) hurriedly arranges Hyun’s discharge and leaves with her. When Shin returns to find the recovery room empty, he’s left deeply uneasy.
Elsewhere, Geum Ba-ra (Joo Se-bin), suspicious about Momo’s seven-month withdrawal from public life, digs into the situation and discovers that Ha Yong-joong (Ahn Woo-yeon) harbors special feelings for Momo. When Geum Ba-ra drinks alone with Ha and hears him claim he’ll “take responsibility” if Shin gives up on Momo, she storms off, upset.
Back from abroad, Shin visits Momo’s home and grows more conflicted as Momo continues to speak like Hyun Ran-hee. Momo (with Hyun’s brain) tells him not to overthink it — “I’ll handle this, it will be fine” — and admits, “It’s confusing, right? When I look in the mirror, I don’t know if I’m Momo or looking at Momo.” Shin, torn between reason and memory, walks away and wrestles with the thought, How could I live without Momo?
Later, when Geum Ba-ra visits the hospital, Shin accuses her of probing into Momo’s affairs. Geum insists she’s worried because she can’t reach Momo; Shin deflects with, “She’s abroad. I told her to rest,” then admonishes Geum — a Nuwa Foundation orphan who he says made her father proud. But just as their meeting ends, news breaks of Shin and Momo’s engagement. Facing backlash at work, Geum turns her conversation with Shin into an exclusive interview. Shin lashes out, “You repay kindness with betrayal?” Geum apologizes, asking him to see it as an attempt to help him grow; Shin cuts her off, “Don’t stab me in the back again. You owe me,” and hangs up.
At home, Momo (with Hyun’s brain) watches Hyun (in Momo’s body) struggle even to eat ice cream. Swallowing her tears, she bathes Hyun, dresses her in a thin slip, and lays her on the bed. Momo turns the air conditioner to its lowest setting, dons gloves, and composes a suicide note designed to make it look as though Hyun suffered clinical depression and took her own life. She folds the note in half and places it beside the bed to stage the scene.
After finishing the note, Momo (with Hyun’s brain) texts Paul Kim (Ji Young-san): “If you find her…tell her mother I’m very sorry…make sure you find her.” A flashback cuts in, showing Paul Kim’s face harden when Hyun arrives to buy a painting, with him muttering, “She hasn’t changed at all,” hinting at a complicated past between Paul Kim and Hyun Ran-hee. As Hyun grows paler from hypothermia, Momo whispers a final farewell: “Hyun Ran-hee lived hard. If this is karma, then it is what it is.”
Later, while Shin is in a meeting with other doctors, his phone rings — it’s Momo (with Hyun’s brain), sounding frantic: “Please come quickly. My mother—” At the same time, Geum Ba-ra, at the police station, hears someone claim that Momo’s mother appears to have taken her own life. Breaking news flashes: “Breaking: Hyun Ran-hee, gallery director, found dead at home; 119 called by Momo.” The bulletin cuts to Ha Yong-joon’s shocked reaction, ratcheting up the episode’s tension.
Episode 4 of TV CHOSUN’s weekend mini-series Doctor Shin airs tonight (the 22nd) at 10:30 p.m. on TV CHOSUN and will stream simultaneously on Coupang Play.
